New Patient Form
Outline of Procedure for New Patients
Step One:
All new patients are requested to fill out a new patient information record. These questions are taken directly from the standard insurance form that we must complete for you. Complete information is required by your insurance carrier.
Step Two:
Discuss your health problem with the doctor.
Step Three:
Step Four:
The doctor will recommend X-rays or any other diagnostic testing only if medically necessary.
Step Five:
The doctor will report his findings, including a review of your examination and X-rays. Together, you will discuss your options for treatment.
Step Six:
Chiropractic techniques, and/or a combination of various rehab and therapy procedures will be utilized to resolve or improve your problem.Your frequency of care will be reduced as your problem improves and you will be released when your condition allows.
